It seems to be scrubbed from the Internet, but the ABC News overnight interview with Holly Hunter, conducted by a fledgling journalist named Merry Miller, was the most painful four minutes on TV.
Miller read even the simplest words off the teleprompter and didn't listen to any of Hunter's answers. Hunter tried to be a pro, but you could see her go from puzzlement to incredulity to wondering if it was a prank.
At the very end, Miller urged people to go to the NBC website to learn more. She was on ABC.

Surprised no one's brought up severely depressed talk show host Christine Chubbuck's infamous on-the-air suicide on Sarasota, Florida's Channel 40.
Per Wikipedia, "On the morning of July 15, 1974, Chubbuck confused co-workers by claiming she had to read a newscast to open [her show] Suncoast Digest, something she had never done before. That morning's talk show guest waited across the studio while Chubbuck sat at the news anchor's desk. During the first eight minutes of her program, Chubbuck covered three national news stories and then a shooting from the previous day at local restaurant Beef & Bottle, at the Sarasota-Bradenton Airport. The film reel of the restaurant shooting had jammed and would not run, so Chubbuck shrugged it off and said on-camera, "In keeping with Channel 40's policy of bringing you the latest in 'blood and guts' and in living color, you are going to see another first—attempted suicide." She drew a revolver and shot herself behind her right ear. Chubbuck fell forward violently and the technical director faded the broadcast rapidly to black. The station quickly ran a standard public service announcement and then a movie. Some television viewers called the police, while others called the station to inquire if the shooting was staged."
Perhaps mercifully no footage of the actual incident survives, but the 2016 movie "Christine" reenacts the scene.

When I turned on the local news soon after, there was live footage of Dornacker being pulled from the water and having CPR performed as she was hauled away on a stretcher. It all happened during the Joey Reynolds Show, on WNNNNBC. (Reynolds had replaced Howard Stern after he was unceremoniously dumped) by WNBC radio. It was the tensest hour of radio I'd ever heard, just Reynolds and the crew trying to put up a brave front the entire time and convincing themselves that it was nothing serious and that she was okay.
That event was so tragic it was the turning point of WNBC radio. The station was probably in financial dire straits anyway but that was the event that probably put the nail in the coffin. Things just were never the same afterward, even though everyone tried to move past it.

Soupy Sales had a live after-school kids show in New York. A regular bit was somebody knocks at his door and they do some bit. The kids didn't actually see the stagehand doing the bit at the door, just Soupy's reactions. Once on Soupy's birthday the producer put more then a stagehand at the door. This went out live to millions of NYC area children BUT the camera shots over Soupy's shoulder were closed circuit only to the control room and Soupy's monitor so he thought this was what was broadcast. . Even in black and white you can see Soupy blushing.

It was an episode of "American Idol" where the contestants each did 2 songs in one show, taking turns (they each did their first song, then in the same order they each did their second song). Paula reviewed a performance that just happened, then for the same singer she went on to say, "For your second song, ..." and went on to give a critique, even though he hadn't sung that one yet. Randy Jackson reminded her of that fact. She was confused because she had watched the rehearsals of both.

Probably tame for this thread's standards, but it made me laugh/smile/raise a brow at the time. To improve my language skills I used to watch a lot of Japanese entertainment television, dumb fluffy stuff about native popstars and filmstars, and during that time came across some weird shit. But the one that stands out to me isn't so much bizarre as it is curious and cringeworthy.
Several years ago in January during a morning interview on NicoNico (eng: 'Smiley Face', a kind of webstream show about popmusic), a teen rock band discuss what they think is funny or strange about their bandmates for the camera. One of them laughingly pipes up that their lead-singer Kenta is "bisexual". Nbd really, even by Japan's standards, except Kenta stares at the floor blushing for a minute after this Reveal and then hilariously scrambles to cover his embarrassed reaction of "WHAT AM I SUPPOSED TO SAY ABOUT THIS?? THIS IS LIVE TV!" (funnier because he's from Osaka and that accent is the Japanese equivalent of a tough-guy Brooklyn/New Yawk accent) by re-enacting the damning evidence his bandmates brought forward (that he likes to sleep next to other guys) to make it seem innocent and platonic. What's interesting is that he doesn't deny being bi or gay, only that he doesn't have sexual designs on his bandmates. Kenta is so thrown by his quasi-outing that he even exclaims that any affection he has for men stems from his father and his overbearing mother. He'd fit right in on the Datalounge.
The segment went from shocking to awkward to silly in the space of 120 seconds, which makes it memorable to me. Japanese men working in mainstream media/entertainment don't really come out or talk frankly about sex, either, so it's a little wild to see (watching it back you notice the female interviewer with them was clearly thrown off-guard by this topic, too). It's a cultural shock that there's still such awkwardness around bi men and that in some parts of the world the idea of m/m intimacy is still a punchline or a way to embarrass someone.
tl;dr I gasped, I laughed, I cried. Get your shit together, Japan.

He wasn't "questioning and rebuking an eyewitness".
